CYSA is requiring all adult volunteers to complete an on-line background check that can be found at http://www.cjsa.net.

There is a box on the right side of the page titled "CJSA Backround Check".  Go to the "Click Here for Background Check" section to submit your information.

Once you have completed submitted your information to CJSA from their site, please e-mail the following to

Team/role:  coach, asst, mgr, and team name;

Name: The same name you use for background check - cannot use "Bill" for the CJSA Background Check and submit "William" to Canton Youth Soccer.
